To use this form you must meet the following criteria: • This form and pricing is only for cemeteries, churches, schools, neighborhoods or civic organization orders. Sorry, no individual orders on this form. • Minimum order of $150 (orders under $150 please use our regular internet pricing.) • Order must be prepaid. No open billing. • Shipping is to a commercial address in the Lower 48 states by our carrier. • No residential deliveries. No International Shipments. UPS as well as most LTL carriers charge more for shipments to residences. If you are ordering for a group and absolutely require the shipment to be delivered to a residence, please contact us before ordering. Be sure to enter quantities in the appropriate price break box. We will correct orders if you enter in the wrong box. (Example: You enter "2" in the 11-24 cs qty. price box. We will move the "2" to the correct box and charge your total accordingly.
